70 year old male with With CVA

This E-blog also reflects my patient's centred online learning portfolio.  A 13years old girl who is a student studying in hostel located at annaram,came with complaints of  Bilateral lower limb swelling since 15days.  HOPI:Patient was apparantly normal 2months back and then she developed nausea and vomitings 4 episodes for 3 days.content food particles.Non bilious.Not associated with pain abdomen.No loose stools.Now since 15 days patient developed bilateral lower limb swelling initially till ankle and now progressed to generalised swelling with puffiness of face. History of migratory joint pains ,1 month back Gross hematuria on the day

HOPI: Patient was apparantly assymptomatic 2 months back and then she developed fever which was insidious in onset,high grade and not associated with chills and rigors and relieved on taking medication and again after one week she again developed fever which is of high grade and 15days back patient developed cough associated with sputum.And her sputum is scanty in amount,white in colour,no blood in sputum and non foul smelling sp

HOPI: Patient was apparently asymptomatic 35years  back and then he developed involuntary moments of upper limbs and lower limbs(?Tonic and clonic moments)for 5 minutes ,and associated with drooling of saliva and loss of consiouness present at the time of those moments.No passage of urine and stools at the time of seizures.Following that episode he went to local hospital for which he was diagnosed as h
